If this is your first stay with us please complete the following prior to your first visit. If you are a return guest please check that you have all of these things up-to-date and on file.

Requirements for boarding your pet with us.

Vaccination Requirements:
Dogs:
Annual DHLP-Parvo Vaccination
Annual or 3 year Rabies Vaccination
Bordetella – every 9 months
Guest Profile(s) (PDF)
Or we will accept proof of acceptable titer count from your veterinarian.

Cats:
Annual Feline Distemper
Annual Rabies Vaccination
Annual Feline Leukemia
Guest Cat Profile(s) (PDF)

It is mandatory that your pet be up to date on all vaccinations prior to staying with us. You are responsible for providing us with proof of your dogs current vaccinations in the form of a print out from your veterinarian. You may either bring this in to us or have it faxed to us prior to your stay. Please understand that for your pets protection and the protection of every other pet staying with us your pet must have all of its vaccinations current, if your pet is not up-to-date on any one of these vaccinations, your pet will not be allowed to stay with us.

Please provide before your stay:

1. Proof of Current Vaccinations.
2. Credit Card on File.
3. Complete & Up-to-date Guest Cat Profile(s) or Guest Dog Profile(s) on file. (PDF)
4. Pet-Fit Boarding and Daycare agreement. (PDF)
5. House rules of Pet-fit Resort (PDF)
